During this session, you will discover how to leverage evidence-based practice and technology to strengthen patient engagement and drive successfully coordinated patient care. You will also have an opportunity to hear firsthand how Altru Health System implemented this approach, and the successes they’ve had along the way.
Learning Objectives: By the end of this educational session, participants will be able to:
- Describe how cross-functional team partnerships can affect your care coordination and patient engagement strategies.
- List ways to implement a framework for collaboration across clinical and technical teams to drive successful patient care coordination.
- Explain how to optimize tools and resources to drive meaningful plans of care and sustain evidence-based, interprofessional practice.
This activity is intended for physicians, nurses, and other healthcare professionals who are interested in increasing their knowledge and competence to enhance care coordination and patient engagement by leverage evidence-based practice and technology.
Donna Mayo, MSN, RN
Senior Director, Informatics
Donna Mayo MSN, RN serves as a Senior Director on the Elsevier Health Informatics team. In her role, Donna provides informatics leadership and collaborates with customers and EHR partners to design and optimize the user experience within the clinical workflow. She is passionate about developing solutions that support the integration of evidence-base practice at the point of care.
Donna earned her BSN and MSN from Grand Valley State University, Allendale Michigan. Donna’s clinical experience includes medical surgical care, telemetry and home health care. She has over 16 years’ experience applying the principles of informatics within the context of clinical practice model framework to advance professional practice and patient centered care.
Renee Olson, LPN
Senior Informatics Analyst
Altru Health System
Renee Olson has more than 40 years of nursing experience, the last 15 of them in an informatics role. She was involved in her organization’s Epic implementation from the selection process through the first “Big Bang” Epic implementation in 2010. Renee continues to build and support the Epic applications for Altru as well as surrounding Critical Access partners. Prior to transitioning to Informatics, she worked in a hospital, nursing home, and an OB/GYN clinic. She has also served on several state and nation al regulatory occupational boards.
Jeff Wieser, MBA
Altru Health System
Jeff Wieser serves as a System Analyst at Altru Health System. In his current role, he supports general Epic Ambulatory functions, including: nurse and provider workflows, Health Maintenance, and BPAs. He uses Care Everywhere to maintain other Epic and non-Epic EHRs at Altru, and is proficient in Epic’s Reporting Workbench, SQL, and Crystal Reports. Since starting at Altru, Jeff has earned 5 Epic certifications and 4 badges, which helped further develop his Epic-related skillset.
Jeff received both his undergraduate degree in Information Systems and MBA from the University of North Dakota. During his undergraduate studies, he held an internship at Altru Health System on the IS helpdesk.
This session will offer 1 nursing contact hour and 1 AMA PRA Category 1 Credit(s)TM, jointly provided by the Elsevier Office of Continuing Medical Education and Elsevier*.
