Most of the provisions of the Copyright Act of
1976 became effective on Jan. 1, 1978. Therefore, all manuscripts must be accompanied by the following written statement, signed by all
authors "The undersigned authors transfer all copyright ownership of the manuscript (title of article) to Mosby, Inc., in the event the
work is published. The undersigned authors warrant that the article is original, does not infringe upon any copyright or other proprietary
right of any third party, is not under consideration by another journal, and has not been previously published. The final manuscript
has been read, and each author's contribution has been approved by the appropriate author." Authors will be consulted, when possible,
regarding republication of their material.
Manuscript preparation
Manuscripts must be typewritten on one side
of the paper only, with double spacing and liberal margins. The abstract should not exceed 150 words typed double spaced on a separate
sheet of paper. If unusual abbreviations cannot be avoided, use the expanded form when first mentioned and abbreviate thereafter. Standard
chemical or nonproprietary pharmaceutical nomenclature should be used. Metric units and the centigrade scale are preferred. Manuscripts
should be limited to 12 to 15 pages. Each page must be numbered.
Illustrations
Original drawings or graphs must
be drawn in black India ink. Typewritten or freehand lettering is not acceptable. All lettering must be done professionally and should
be in proportion to the drawing, graph, or photograph. Do not send original artwork, x-ray films, or ECG tracings. Glossy print photographs,
3 x 4 inches (minimum) to 5 x 7 inches (maximum), with good black-and-white contrast or color balance are preferred. Consistency in size
within the article is strongly preferred. Any special instructions regarding sizing should be clearly noted. Electrocardiograms are notoriously
difficult to reproduce adequately, ECG paper smudges easily, so great care must be exercised in the preparation of a glossy print. Unless
a dysrhythmia is to be illustrated, one or two complexes should be photographed showing only those leads necessary to document the conclusions.
A longer strip of a particularly illustrative lead may be used for dysrhythmias. All illustrations must be numbered and cited in the
text. Typed legends must accompany each illustration and be listed on a separate page. Each illustration should have the following information
only lightly penciled on the back or typed on a gummed label affixed to the back of the figure: figure number, title of manuscript, and
an arrow indicating the top of the figure. Do not use paper clips to secure illustrations because they can damage the surface. Place
illustrations in a separate envelope and mail with the text. A letter of permission must accompany all photographs of persons when there
is a possibility of identification. Illustrations will be returned only if requested by the author. Special arrangements must be made
with the Editors for color plates, elaborate tables, or extra illustrations.
Electronic illustration submission
Figures
may be submitted in electronic format. All images should be at least 5" wide. Images should be provided in EPS or TIF format on Zip disk,
CD, floppy, Jaz, or 3.5 MO. Macintosh or PC is acceptable. Graphics software such as Photoshop and Illustrator, not presentation software
such as PowerPoint, CorelDraw, or Harvard Graphics, should be used in the creation of the art. Gray scale images should be at least 300
DPI with a proof. Combination of gray scale and line art should be at least 1200 DPI with a proof. Line art should be at least 1200 DPI
with a proof. Please include hardware and software information, in addition to the file names, with the disk.
Tables
Tables should be self-explanatory and should supplement, not duplicate, the text. Tables must be numbered consecutively and mentioned
in the text, and each must have a title. Each table must be typed with double spacing on a separate sheet.
References
References must be cited consecutively in the text as superscript numerals. If the same reference is cited more than once, the original
numeral is used. References should be typed, double spaced, on a separate sheet in numeric order. References to journal articles must
contain, in order, the following information: authors' names (last names, ititials); title of article (initial capital and lower case,
no quotation marks); journal name abbreviated as in Cumulated Index Medicus; and year of publication, volume number, and inclusive page
numbers. If six or fewer authors, list all; if seven or more, list first six and et al. An example follows:

References to
books must include names of authors, book title (not italicized), volume number (if any), edition number (if any), city of publication,
and, if applicable, pages cited. Examples follow:

2nd ed. St Louis: CV Mosby; 1985. p. 10, 12, 484-6.
For books with editors listed, cite as follows:

Note: Personal communications should not be included in the list of references but may be cited in the text in parentheses.
Manuscript editing
Manuscripts become the permanent property of
Heart & Lung®
and may
be be published elsewhere without written permission from the author and Journal publisher. All accepted manuscripts are subject to manuscript
editing.
Animal experimentation
Animal experimentation must observe the "Guiding Principles in the Care and Use
of Animals" published in the
Guide to Authors of the American Physiological Society, and be approved by the appropriate Animal
Care Committee.
Letters to the Editor
Letters to the Editor raising some point of current interest or commenting
on an article that appeared in the Journal will be considered for publication. The Editor reserves the right to accept, reject, or excerpt
letters without changing the views expressed by the writer. The author of an original article will have an opportunity to reply to published
comments. All such letters should be sent to Dr. Stone.
Announcements
Meeting announcements must be received by
Mosby at least 2 months before publication (see Educational Directory). Such announcements should concern major meetings and other significant
activities.
