ICD-10-CM Coding and Documentation of Heart Failure and Pulmonary Hypertension

This webinar will review recent updates to ICD-10-CM heart failure and pulmonary hypertension codes and guidelines.  It will address essential knowledge required for coders and CDI specialists to identify the documentation necessary to support heart failure and pulmonary hypertension diagnoses, and facilitate accurate coding and reporting. Time will be devoted to briefly explaining the impact of codes and sequencing on reimbursement and CMS’s quality programs.

  • Define heart failure and pulmonary hypertension diagnoses, noting underlying causes, certain associated conditions, and potential complications
  • Identify clinical characteristics of heart failure and pulmonary hypertension as documented in the medical record
  • Recognize documentation requirements to facilitate complete and accurate ICD-10-CM codes
  • Apply official coding and sequencing guidelines for compliant billing and reporting

Presenters

Beth Ford, BAS, RHIT, CCS, is a Content Manger and subject matter expert for Elsevier's Clinical Solutions Division. She has 30 years of experience in physician and facility coding and compliance. Beth also has extensive experience in a variety of heath care settings, including acute and post-acute facilities, occupational health, and ambulatory care. She has provided coding education and consulting services to hospitals and physician practices and has developed curriculum for medical terminology and coding education for large healthcare systems and multi-specialty physician groups. Formerly, Ms. Ford served as a technical writer and editor, coding trainer/educator, coding manager, and health information management director. She is credentialed by the American Health Information Management Association (AHIMA) as a registered heath information technologist (RHIT) and certified coding specialist (CCS). She is an active member of the AHIMA and is an AHIMA-Approved ICD-10-CM/PCS trainer.

Lynn-Marie Wozniak. MS, RHIT, is a Revenue Cycle Content Manger and subject matter expert for Elsevier's Clinical Solutions Division. Lynn has over 30 years of experience in hospital reimbursement consulting, health services research, and health policy review and development.  Ms. Wozniak is a past president of the New York Health Information Management Association (NYHIMA), as well as a past commissioner of the New York Department of Health’s Data Protection Review board, and AHIMA's Commission on Certification for Health Information and Informatics Management (CCHIIM). She has also served on AHIMA’s Council on Accreditation, research review panel, and program, nominating, and scholarship committees. She is a recipient of AMRA’s (now AHIMA) 1989 Professional Achievement Award, and the 1993 NYHIMA Distinguished Member Award.
